Seaman Health Services: Medication/ Treatment/ Procedure

Students may need medications (prescription/over the counter, emergency) to participate in their education program.  These students shall receive medication as an accompaniment to their educational program.  ALL medications are to be brought to the healthroom and  assessed by the school nurse before being administered in the school setting.

The Seaman USD #345 school district medication policy states that medication will be administered following these guidelines:

  1. "Seaman Request and Permission for Medication at School""Seaman Request and Permission for Medication at School" form must be signed by a parent or guardian for all prescription and over the counter medications.
  2. All prescription medications must have a current pharmacy label which clearly states the name of the authorized prescriber, student's name, the name of the medication, dosage, time(s) and route to be administered.
  3. All non-prescription medication must be provided by the parent in its original container and authorized for the dosage recommended on the package label for children unless the medical provider provides written permission.
  4. The medical need for self-managed administration  of medication must be evaluated on an individual basis by the school nurse.  Controlled substances may not be self-administered.  The "Seaman USD #345 Permission to Self-Administer Medication" form must be completed with written instruction from the physician, parent, school nurse and student.

Students with chronic disease or acute illness may require treatments or procedures during the school day.  All treatments and procedures must be ordered by a physician.  Click here to download and print a "Seaman USD #345 Health Services Physician Authorization for Treatment Procedure at School".
