Required Health Forms

In order to protect the health and well-being of the students and staff in our schools, it is important for parents and school nurses to maintain open communication and collaboration.  Please keep your school nurse updated on your child's health status, such as surgeries, medications, illness/disease, new diagnosis, immunizations, and/or other conditions that may affect your child's academic performance.

Health Assessment - According to KSA 72-5214, any student new to Kansas schools up to age nine years must provide the school with documentation of a health assessment conducted within the last 12 months by a public nurse, a physician, or by a person acting under the direction of a physician.  Chiropractor cannot perform these health assessments.  Seaman USD #345 Youth Health Assessment form, this can be downloaded and completed  by your healthcare provider.

Immunizations - Written documentation of completed mandated immunizations against certain childhood diseases is required for all students preschool through twelth grade.  Students who are not in compliance with Kansas law will be excluded from school.  Click here to view and download specific Immunization Information for the 2010-2011 school year. 

Health History - Health histories are completed at enrollment or sent home by the school nurse early in the school year.  The health history is a document that is completed by the parent/guardian to assist the school nurse with identifying students with chronic illness or special health care needs.  The health history also provides demographic information on each student, such as health insurance status, last visit to physician, dentist or optometrist, and emergency contact information.  The health history is filed in the student's health folder and locked in a cabinet in the health room.  Links for Health History forms are shown below.
